Longevity Medicine Consultations

Dr Agnes Warchalowski

Longevity Medicine is an exciting and emerging field of medicine based on research into the cellular and genetic mechanisms that contribute to ageing. This research is identifying compounds, tools and techniques that can prevent, delay and in some cases, even reverse the signs and cellular damage associated with ageing.

The key areas of research include:

  • Cellular repair and renewal
  • Energy production and utilisation
  • Mitochondrial function
  • Genetic stability
  • Epigenetic signalling
  • Antioxidant pathways
  • Immune system optimisation
  • Metabolic flexibility
  • Hormonal balance
  • Neurogenesis and
  • Autophagy (cellular recycling)

Longevity protocols incorporate a range of tools and techniques that can optimise these key processes and therefore may assist with certain pathological states such as:

  • Accelerated ageing; skin ageing, hair loss and muscle loss.
  • Loss of barrier integrity; leaky gut and leaky brain.
  • Immune dysfunction; food intolerances, autoimmune processes and allergies
  • Skin conditions; acne, dermatitis, rosacea and psoriasis
  • Metabolic disruption; insulin resistance, weight gain and carbohydrate intolerance.
  • Hormonal imbalances; adrenal dysfunction, thyroid disorders, polycystic ovaries and menopause.
  • Neurodegenerative and neuroinflammatory changes: mood disorders, dementia, cognitive decline and potentially, addiction.

It's important to understand that everyone differs in their physiology, their past, their environmental exposures, where they've been, what they've experienced, their lifestyle, family and work, their past and current health practices, and even their use of medical services and treatments - all of these influence the choice of tools used in longevity protocols. This is why a tailored plan is essential.

Dr Agnes now offers individual  Longevity Medicine Consultations, during which she assesses your past and current history and discusses methods, tools, techniques and compounds that can help address your anti-ageing needs.

How to book a Longevity Medicine Consultation

All patients must first complete a Health Questionnaire and return it along with any recent blood tests.*

You must then book an Initial Health Consultation, during which Dr Agnes will briefly assess your questionnaire and provide guidance on whether it is best to start with one of our health programs (BiomeMD or the hCG Diet) or book a Longevity Medicine Consultation.

*Please note: Dr Agnes only refers for privately-billed functional tests. It is therefore recommended that you see your GP for a general blood test and forward the results prior to your initial appointment with Dr Agnes in order to avoid extra costs.

These are the tests your GP may perform:

  • Full blood count
  • Tests for liver, kidney and thyroid function
  • Fasting lipid, cholesterol and blood sugar
  • Cortisol (stress hormone)
  • Vitamin and mineral levels: B12, folate, B6, iron, Mg, Cu, Iodine, Zinc
  • Inflammatory markers: CRP, ESR
  • And any others as directed by your symptoms

What to expect from Longevity Medicine Consultations

Longevity Medicine Consultations are 60 minutes each, and you will need at least three consultations to develop the complete protocol.

The consultations will proceed as follows:

  1. First Consultation – a thorough assessment of your history, advice regarding testing and medication management and provision of a personalised nutrition plan (more information below).
  2. Second consultation – an assessment of progress and results, discussion of individualised lifestyle techniques, advice regarding further testing and medication management as well as tools for implementing techniques and monitoring progress (more information below).
  3. Third consultation – a review of your progress, discussion of supplements and compounded formulations, as well as tools and treatments for assisting underlying processes (more information below).

Here are more details of what to expect at each consultation.

The First Consultation

Dr Agnes will explore all areas of your life that may be affecting your health or keeping you stuck. The aim is to identify what needs to change in order to prevent or delay cellular decline, or reverse cellular damage where possible.

This consultation focuses on dietary modification as the first crucial step for optimising health. How effective a particular dietary plan will be, depends entirely on your physiology - and everyone is different in that respect. By taking a thorough history, Dr Agnes can identify the dietary factors that are affecting your health and potentially contributing to premature ageing. These factors include - but are not limited to:

  • How well your body tolerates carbohydrates, gluten, dairy, processed and fermented foods
  • Whether you suffer from underlying food intolerances
  • To what degree sugar is contributing to symptoms such as hot flushes, wrinkles and fluid retention
  • The nature and timing of your cravings.
  • Whether you have features of pre-clinical or sub-clinical thryoid disease, hypothalamic and adrenal issues or insulin resistances.
  • The effect of certain foods on your sleep cycles
Dr Agnes will then determine which dietary techniques are safe for you and best suited to your needs. She will also discuss how to set out your meals mid-week, on weekends and on or after special occasions.

The techniques that Dr Agnes uses are usually implemented in cycles and depend entirely on your physiology. Your plan may include some or all of these techniques:

  • SAFE intermittent fasting
  • SAFE and intermittent ketosis
  • Circadian rhythm timing
  • Carbohydrate cycling
  • Fasting-mimicking diet cycles
  • Liquid fasts (NOT water fasting).

These techniques have proven anti-ageing and regenerative benefits if they are implemented in a way that works with your physiology. Unfortunately, there is much misinformation on these techniques, and professional guidance is essential to achieve the best results.

At the end of this Consultation, Dr Agnes:

  1. Will provide you with a written personalised nutrition plan which forms the first step of your protocol.*
  2. May suggest more testing through your GP as needed.
  3. May refer you for private functional tests.
  4. May make recommendations regarding medication management or modification which you can discuss with your GP
  5. We will provide resources to assist you with implementing the dietary techniques. These may include behavioural plans, educational videos or safe food brands.**

*This plan includes information on the dietary techniques that are best for you as well as a food list, meal timing, macronutrient proportions and information regarding how and when to use specific dietary techniques. The plan will not have everything discussed during the consultation, particularly if modifications are needed, so Dr Agnes will ask that you take your own notes.

**Please note that we do not receive any incentives for any recommendations made.

The Second Consultation

Dr Agnes will assess your progress so far and make any tweaks as needed. She will also assess the results of any tests (if ordered) and then discuss individualised tools to address sleep, mind management, exercise and lifestyle. Techniques must be chosen according to your physiology, as well as your daily, weekly and monthly patterns.

Sleep research has revealed benefits that extend to every aspect of life. Sleep is the time when your brain consolidates memories. It is also when immune cells within the brain remove the junk that has accumulated during the day. Without adequate sleep, this brain-cleansing process is impaired. There is a clear link between sleep disorders and brain disorders such as neurodegenerative conditions and mood disorders. At the very least, poor sleep can impact mood and decision-making which in turn affects all aspects of life.

Similarly, factors such as poor stress management or inappropriate exercise can have a significant and multifactorial impact on your health, leading to accelerated ageing. For example, yoga may be great for some but may aggravate long-standing injuries in others. And when it comes to exercise, it must be appropriate to your physical capacity. As example, high-intensity exercise boasts many health benefits; however, it can elevate stress hormone levels and therefore should not be practised if you have adrenal or thyroid problems.

Dr Agnes will identify the molecular mechanisms that influence your sleep, stress management and exercise capacity. These mechanisms are unique to your physiology and can be modified where needed. The aim is to guide the body towards following natural circadian rhythm cycles to optimise sleep, energy levels, mental focus and your ability to manage stress.

You will be provided with a simple plan that lists the tools and techniques you can implement at certain times of the day to take advantage of natural circadian cycles.

You will also need to take your own notes as a substantial amount of information will only be given verbally. However, you can opt for a word document protocol that can be provided at an extra fee that is time-dependent according to the complexity of your needs.

At the end of this Consultation, Dr Agnes:

  1. Will provide you with a basic plan that lists the tools and techniques you can incorporate at particular times each day to improve your energy levels and get a good nights sleep.
  2. Will provide resources to assist you with implementing the techniques discussed. Most of these resources are free; however, some may incur a fee - such as meditation apps or sleep trackers.*
  3. May suggest more testing, treatment or medication changes that you can discuss with your GP
  4. May refer for functional tests.

*Please note that we do not receive any incentives for any recommendations made.

The Third Consultation

This consultation is to assess your progress so far and address any areas that are not adequately managed with the tools above. Dr Agnes will develop an individualised program that may include compounded nutrient formulations, vitamin injections or infusions, a novel antioxidant, anti-inflammatory and anti-aging compound/s, hair loss formulations and personalised compounded skincare. She may also recommend evidence-based treatments or products such as acupuncture, LED caps for hair loss or infrared panels to reduce inflammation.

Inflammation is the underlying cause of many symptoms and conditions. It is also the cause and consequence of ageing - a link known as inflammageing. For some people, an individualised diet and lifestyle plan is sufficient to manage inflammation. Others, however, may require anti-inflammatory supplements or prescription compounds. Many compounds have significant immune, neurological and anti-ageing benefits. Dr Agnes will determine which ones are best suited to your needs and tailor a list that addresses your needs.

You will need to take your own notes during this consultation as a substantial amount of information will only be given verbally. However, you can opt for a word document protocol that can be provided at an extra fee that is time-dependent according to the complexity of your needs..

At the end of this Consultation, Dr Agnes:

  1. Will provide a basic plan for specific supplements, services, treatments or products*.
  2. May provide prescriptions for compounded products*
  3. Provide written resources on where to purchase certain compounds and products or obtain treatments such as vitamin infusions.*
  4. May suggest more testing, treatment or medication changes that you can discuss with your GP
  5. May refer for functional tests

If your plan is complete, it is recommended that you book a Review Consultation in 1 month.

If, however, your individual needs are more complex, Dr Agnes will suggest a further 60-minute consultation/s to complete your plan.

*BioAesthetics may receive a marginal incentive for some of the recommended products however, please note that these recommendations are not guided by incentives but rather by evidence of efficacy.
